Assumption of Risk, Release and Permission
Participation in the 2026 Walk for Talk may involve physical activity and inherent risks, including, but not limited to, falls, contact with other participants, weather conditions, traffic, trail or roadway conditions, and other hazards. By participating in this event, I voluntarily assume all risks associated with my participation, including bodily injury, illness, death, property damage, or other loss arising from or related to my participation in the Walk for Talk and its related activities.
I acknowledge that I am solely responsible for my own health and safety and represent that I am physically able to participate in this event.
In consideration of being permitted to participate, I, on behalf of myself and my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and agree not to bring any claim against Dysphonia International (formerly the National Spasmodic Dysphonia Association), its support groups, officers, directors, employees, volunteers, sponsors, agents, successors, and assigns from any and all liability, claims, demands, or causes of action arising out of or related to my participation in or attendance at the Walk for Talk or related activities, including claims resulting from the negligence of the released parties, to the fullest extent permitted by applicable law. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
Photography and Media Release
I grant Dysphonia International permission to photograph, film, record, or otherwise capture my likeness, name, voice, and image during the event and to use, reproduce, publish, distribute, and display such materials in perpetuity, without compensation, in any media now known or later developed, including but not limited to printed publications, websites (including dysphonia.org and walk4talk.org), social media platforms, fundraising materials, and other promotional or educational communications.
Minor Participants
If the participant is under 18 years of age, the participant's parent or legal guardian must accept this waiver on the participant's behalf. By registering a minor, the parent or legal guardian represents that they have the legal authority to do so, consents to the minor's participation, and agrees to all terms of this waiver on the minor's behalf.
Event Changes
Dysphonia International reserves the right to postpone, modify, or cancel the event due to weather, public safety concerns, or other circumstances beyond its control. Registration fees and donations are non-refundable unless otherwise determined by Dysphonia International.
Acknowledgment
By submitting this registration, I acknowledge that I have carefully read and understand this Assumption of Risk, Release and Permission and agree to be bound by its terms. If the participant is under 18 years of age, this acknowledgment must be made by the participant's parent or legal guardian.
